I, Mohan, son
of Chanmougame, aged 62 years,
residing at Old No. 9/15, New No. 8, Nalla Thannier
Kinaru Street, Kuyavarpalayam, Puducherry-605 013,
do hereby solemnly declare and affirm on oath as
follows:
I state that I am the deponent herein and I am well
aware of the facts of this deposition.
I state that in my Birth Certificate under
Registration No. P/1953/01208, dated 3-6-1953 issued
by the Registrar of Births and Deaths, Puducherry
Municipality, Puducherry, my name was mentioned as
‘Madoure alias Mohane’ and whereas, my name has
been entered as ‘D. Mohan’ in my Aadhaar Card
No. 4314 0500 2776 issued by Unique Identification
Authority of India, Government of India and also
I state that my name was given as ‘Madourai alias
Mohane. C.’ in my Provisional Certificate, Bachelor
of Laws vide Register No. 2619423 issued by
Pondicherry University, Puducherry and in the
Bar Council of Tamil Nadu and Puducherry Certificate
of Enrolment as Advocate Roll No. 2653/2013, dated
21-12-2013 issued by the Bar Council of Tamil Nadu
and Puducherry and as ‘Mohane. C.’ in my Master
of Law Provisional Certificate bearing Register
No. 058M0635P00239 issued by Annamalai University,
Annamalai Nagar, Chidambaram.
I further state that in my Voter’s Identity Card
No. PY/01/007/000001 issued by Election Commission
of India and in my PAN Card vide Card No. AIEPM0231H
issued by Income Tax Department, Government of
India, my name was entered correct spellings as
‘Mohan’.
I state that the names ‘ Madoure alias Mohane’,
‘D. Mohan’, ‘Madourai alias Mohane. C.’, ‘Mohane. C.’
and ‘Mohan’ refer to one and the same person, i.e.
myself only.
I state that my correct name and my name
henceforth be known as ‘Mohan’ only.
I further state that I made this affidavit in lieu of
proof of identity of my names.
Hence, this sworn affidavit may kindly be accepted
to this effect.
I take oath and solemnly declare/affirm that the
particulars furnished by me above are correct and
that I have not concealed or misrepresented any
facts.

By this deed I, the undersigned Jayabal (new
name), etc., called Jayabalan.V. (old name), son of
Viswanathan, aged about 33 years, auto driver and
residing at No. 40, Middle Street, Kanabathichettikulam,
Puducherry, solemnly declare:
1. That for and on behalf of myself and my wife,
children and remitter issue, I wholly renounce/
relinquish and abandon the use of my former name/
surname of ‘Jayabalan. V.’ and in place whereof,
I do hereby assume from this date the name/surname
‘Jayabal’ and so, that I and my wife, children and
remitter issue may hereafter be called, known and
distinguished not by my former name/surname,
but assumed name/surname of ‘Jayabal’.
2. That for the purpose of evidencing such my
determination declare that I shall at all times
hereinafter in all records, deeds and writings and
in all proceedings, dealings and transactions,
private as well as upon all occasions whatsoever
use and sign the name of ‘Jayabal’ as my name/
surname in place and in substitution of my former
name/surname. This name change is made for the
purpose of numerology.
3. That I expressly authorise and request all
persons in general, relatives and friends in particular
at all times hereafter to designate and address me,
my wife, my children and remitter issue by such
assumed name/surname of ‘Jayabal’ accordingly.
4. In witness whereof, I have hereunto subscribed
