I, Vidjeacoumar, son of Periapaquirissamy @ Paquirissamy,

aged about 53 years, residing at No.1, Siddhi Vinayakar

Koil Street, Vanarapet, Puducherry-605 001, do hereby

solemnly and sincerely affirm as follows:

That I am the deponent herein.

That my correct name is ‘Vidjeacoumar’ as per my

Birth Certificate, my son’s Birth Certificate, my son’s

Aadhaar Card and Voter’s Identity Card, etc.

But, my name is wrongly mentioned in my son’s

Driving Licence issued by the Transport Department,

Puducherry (D. L. No. 20054336) as ‘Vijayakumar’.

I hereby declare that the names ‘Vidjeacoumar’ and

‘Vijayakumar’ are one and the same person i.e.,

myself only.

And in future I may be called only as ‘Vidjeacoumar’.

That the above matter is also true and correct to

the best of my knowledge and belief.
