and state on oath and declare as follows:

That I am the deponent herein and well aware of

the facts of the deposition.

That in my Birth Certificate registered under

Registration No.O/1979/1218 with the Oulgaret

Municipality, in my Electoral Identity Card under

No. GKY0413583 issued by the Election Commission

of India and in my daughter Ramya’s Certificate of

Birth registered under Registration No. M/2009/01989

registered with the Puducherry Municipality, my name

has been mentioned as ‘Raja’.

Whereas, in my family Ration Card under

No.359861 issued by the Department of Civil Supplies

and Consumer Affairs, Puducherry, my name has

been mentioned as ‘Selvaraj’.

That the said names ‘Raja’ and ‘Selvaraj’ are refer,

identify and relate to one and the same person that

is myself, the deponent herein. I shall henceforth be

known and identified by the name ‘Raja’ only for all

purposes.
