CHANGE OF NAME

I, Bakkiavathy @ Vathi, wife of Ramamoorthy, aged

43 years, residing at No. 23, Sengeniamman Koil Street,

Solai Nagar, Muthialpet, Puducherry-605 003, do hereby

solemnly and sincerely affirm on oath and declare as

follows:

I state that I am the deponent herein and I am well

aware of the facts of this deposition.

I state that I am living at the abovesaid address

along with my family members and I state that in my

Family Ration Card vide No. 181163, my name was

mentioned in Tamil mistakenly as‘’ instead of

‘’.

I state that my name has been mentioned correctly

as ‘Bakkiavathy’ in my son’s Birth Certificate which

is registered at Puducherry Municipality, Puducherry

under Registration No. M/1992/07287, dated

7-8-1992 and whereas, my name was entered in Tamil

mistakenly as ‘’ (Vathi) in the Voter’s Identity

Card No. IVT0055517 issued by Election Commission

of India, in my Family Ration Card No. 181163 issued

by Department of Civil Supplies and Consumer

Affairs, Puducherry and in my Aadhaar Card

No. 6018 7198 9328 issued by Unique Identification

Authority of India, Government of India.

I state that the names ‘Bakkiavathy’ and ‘’

(Vathi)

refer to one and the same person, i.e. myself

only.

I state that my correct name and my name

henceforth be known as

‘Bakkiavathy ()’

only.

I state that I made this affidavit in lieu of proof

of my names and this affidavit is to be produced

before the Department of Civil Supplies and

Consumer Affairs, Puducherry for making necessary

changes in my name as ‘’ instead of ‘’ in

my Family Ration Card No. 181163.

I further state that in case of any dispute arises

in this regard in future, I will solve the problem at

my own risk and costs.

I take oath and solemnly declare/affirm that the

particulars furnished by me above are correct and

I have not concealed or misrepresented any facts

thereon.
