(4) I swear and state that I intent to change my

name as ‘Naseema Kassim’. Therefore I swear this

affidavit for knowledge of my relatives, friends,

officials concerned and others that hereinafter I will

be known only as ‘Naseema Kassim’ in all purposes

and intends after the publication of the Gazette.
