CHANGE OF NAME

I, Shenbagavalli Balasundaram, residing at Durga

Illam, No.1, 1st Cross Street, Kuppampet, Kombakkam,

Villianur-605 110, Puducherry, do hereby solemnly and

sincerely affirm on oath and declare as follows:

I am the deponent herein.

My name found in my Voter’s Identity Card as

‘Shenbagavalli Balasoundirane’, found as ‘Shenbagavalli’

in my Family Ration Card refer to one and the

same person, i.e. myself. Hereinafter my name shall be

called as ‘Shenbagavalli Balasoundirane’.

I hereby declare that the abovesaid facts are true

to the best of my knowledge and belief and

information.

Solemnly and sincerely affirmed and signed
