as ‘Kumar’ in the Family Ration Card. My name stands

as ‘Jayakumar’ in the College Transfer Certificate and

Mark Sheet. My name stands as ‘Jayakumar’ in the

Voter’s Identity Card, my name stands as ‘Jayakumar’

in the Aadhaar Card and PAN Card. My name stands

as ‘Jayakumar’ in my Driving Licence.

2. Therefore, I further affirm that above said names
