I, Jayarakini, daughter of Antoni Muthu, Christian
aged 35 years, residing at No. 3, Anthoniar Koil Street,
Dubrayapet, Puducherry-605 001, do hereby solemnly
and sincerely affirm and state as follows:
I am the deponent herein and I am well acquainted
with the facts of my deposition.
I state that my name is mentioned as ‘Jayarakini’
in my Birth Certificate issued by
Puducherry

dated 3-8-1978 and in my Transfer Certificate issued
by the Sinnatha Government Girls’ Higher Secondary
School, Muthialpet, Puducherry, vide Serial No. 072257
(Admission No. 4518).
I state that my name is mentioned as ‘Jayarakini
Antoni Muthu’ in Office Memorandum issued by
Department of Personnel and Administrative Reforms
(Personnel Wing), Government of Puducherry vide
No. A.12011/4/2014-DP&AR/GC/UII/P(HK), dated 15-7-
2014. In that my name is mentioned along with my
father’s name.
I finally state that my above names ‘Jayarakini’
and ‘Jayarakini Antoni Muthu’ are one and the same
person, i.e. myself only.
This affidavit is made in lieu of proof of my
different names ‘Jayarakini’ and ‘Jayarakini Antoni
Muthu’ are refers to same and one person it denotes
myself only.
