CHANGE OF NAME
I, Ibrahim, son of Abdul Ali, aged 62 years, Muslim,
Employee, residing at No. 3, Sumaya Nagar,
Dharmapuram, Karaikal, do hereby solemnly affirm and
sincerely state as follows:
My name is ‘Ibrahim’. In all my records relating
to me such as my Family Ration Card No. 442967,
Electoral Identity Card No. CHD0245191 and Aadhaar
Card 8239 1197 2891 my name is wrongly entered as
‘Mohamed Ibrahim’. But in my son’s Mohamed
Jahabar Ali’s Birth Certificate my name is correctly
entered as ‘Ibrahim’. I hereby give up the name
‘Mohamed Ibrahim’ and I shall henceforth be known,
called, identified and referred only as ‘Ibrahim’.
Solemnly affirmed and signed before the Notary Public
