dated 14-5-2003 and in my Bank Passbook issued
by Primary Agricultural Co-operative Bank, Korkadu,
Puducherry. Whereas, in my Family Ration Card my
name is wrongly mentioned by mistake as ‘Gayathiri’
and the said incorrect name crept in my
Family Ration Card vide No. 014326 due to
inadvertence.
4. I submit that mentioning of my name wrongly
as ‘Gayathiri’ in my above Family Ration Card
is causing unnecessary confusion and hardship to
me. Hence, I am in dire need of rectifying in my
Family Ration Card by entering my correct name
‘Savithiri’ and by deleting my incorrect name
‘Gayathiri’.
I affirm and state that I make this affidavit in lieu of
