Munsifi, do hereby solemnly affirm and sincerely oath

as follows:

I do submit that I was born at Karaikal on 24-5-1981

and my birth was duly registered with the Karaikal

Municipality. As per my Birth Certificate, my name

is mentioned as ‘Sourirajan’.

I have changed my name as ‘P.S. Souriraajan’ from

the original name of ‘Sourirajan’ based on the

numerology for my bright future in all aspects.

I do affirm that I shall hereinafter be called and

known as ‘P.S. Souriraajan’ for all purposes.

I am swearing this affidavit for the knowledge of

my relatives, friends, general public and the entire

private and government organisations to know

that I shall hereinafter be called and known as

‘P.S. Souriraajan’.

I declare that the above facts are true to the best

of my knowledge, belief and information and nothing

has been concealed thereon.
