I, B. Kalaiselvi, wife of B. Balu, aged about 31 years,

residing at No. 38, Vanidasan Street, Kuyavarpalayam,

Puducherry-605 013, do hereby solemnly and sincerely

affirmed on oath as follows:

I am the deponent herein.

I declare that my correct name with spelling is

‘Kalaiselvi’. The same correct name is correctly

entered in my 9th Standard School Transfer Certificate

Serial No. 37/99-2000, Admission No. 4371 issued by

the Government of Tamil Nadu, Department of School

Education and in my Aadhaar Card No. 2810 9730 3984

issued by the Unique Identification Authority of

India.

I further declare that my calling name is ‘Selvi’ in

my home. I further declare that instead of my correct

name with spelling ‘Kalaiselvi’ is erroneously entered

as ‘Selvi’ into my Voter’s Identity Card No. SCO0017293

issued by Election Commission of India and in my

Family Ration Card No. 163364 issued by the

Department of Civil Supplies and Consumer Affairs,

Government of Puducherry. I declare that both the

names ‘Kalaiselvi’ and ‘Selvi’ are refer to me only.

I further declare that I am making this affidavit for

the purpose of change of my name as ‘Kalaiselvi’ into

my Voter’s Identity Card and my Family Ration Card.

The above deposed paragraphs are true to the

best of my knowledge, belief and information and

nothing has been concealed therein.
