within fifteen days from the date of publication of this notice in the official gazette.

The objections so received within the specified period will be considered by this Commune Panchayat.

I, Saroj Devi, wife of Govindlal, aged 61 years (Date

of birth: 8-1-1953), residing at No. 49, First Floor, Perumal

Koil Street, Puducherry-605 001, do hereby solemnly

declare and affirm on oath as follows:

I state that I am the deponent herein and I am well

aware of the facts of this deposition.

I state that my name has been mentioned in correct

spelling as ‘Saroj Devi (’ in my Aadhaar Card

No. 7879 7214 6376 issued by Unique Identification

Authority of India, Government of India and in my Voter’s

Identity Card No. PY/01/002/054188 issued by Election

Commission of India, and the said name is written in Tamil

as ‘’ in the Elector’s list. I further state that my

name has been entered mistakenly in Tamil as ‘’

in our Family Ration Card No. 004448 issued by Department

of Civil Supplies and Consumer Affairs, Puducherry.

I state that my names ‘Saroj Devi (’,

‘’ and ‘’ are refer to one and the same

person, i.e., myself only.

I state that my correct name and my name henceforth be

known as ‘’ only.

I further state that I made this affidavit in lieu of proof of

my name, which is to be produced before the Department of

Civil Supplies and Consumer Affairs, Puducherry for making

necessary changes in my name as ‘’ instead of

‘’ in our Family Ration Card .
