solemnly and sincerely affirm the following facts:

I am the deponent herein.

That I have converted from Christian community

to Muslim community.

That I have changed my name from ‘Kalmeena

Mary’ to new name of ‘Afreena’. Hereafter, I will be

is ‘M. Vettrivel’ as such in my Aadhaar Card under

No. 4099 2362 7925, my name has been mentioned as

‘M. Vettrivel’.

Further, in my PAN Card No. BONPS3560P, my name

has been mentioned as ‘Vettrivel M Sengunthar’. I further

state that in my academic certificates, my name has been

mentioned as ‘Vettrivel. M’.

Whereas, in my Elector’s Photo Identity Card

No. JTP0545756, my name has been mentioned with

spelling as ‘Vetrivel’. Similarly in my Family Ration Card

under No. 075105, my name has been mentioned

as ‘Vettrivel’ in Tamil language.

In conformity with rule 8 of the Puducherry Municipalities (Grant of Licences and Permissions)

Rules, 1976, promulgated in this Union Territory by the Puducherry Municipalities Act, 1973, objections, if any, to the setting

up of the above industry, are invited to reach the Commissioner, Karaikal Municipality within ten days from the date of

publication of this notice in the official gazette.

The objections so received within the specified time will be considered by the Karaikal Municipality in accordance with

the rules.
