appointment issued by the Office of the Deputy

my name has been entered as ‘Rupa. S’.

The initial ‘K’ refers my father’s name Karmegham

and the initial ‘S’ refers my husband’s name

Sedouramane.

Both the names ‘Rupa. K’ and ‘Rupa. S’ mentioned

in the above documents are refer to one and the same

person i.e., myself.

follows:

I am the deponent in this affidavit and well

acquainted with the contents of the same.

I was born on 15-11-1980 to my parents Selvaraj

and Thilagam at Iyyankuttipalayam, Kurumampet,

Puducherry and the same was not registered due to

my parents inadvertence. I studied 9th Standard in

the Ilango Adigal Government Higher Secondary

School, Muthirapalayam, Puducherry and as per my

Transfer Certificate my name is ‘Karthiga’. In my

Family Ration Card vide No. 308570 issued by the

Department of Civil Supplies and Consumer Affairs,

Government of Puducherry, Voter’s Identity Card vide

No. GKY0395186 issued by Election Commission of

India and Aadhaar Card vide No. 4898 4783 2646

issued by Government of India, my name has been

entered as ‘Lakshmi’. So, the above names ‘Karthiga’

and ‘Lakshmi’ are refer to one and the same person

i.e., myself. Hereafter I must be known as ‘Karthiga

@ Lakshmi’.
