CHANGE OF NAME

I, K. Amudha, wife of M. Kamaraj, aged 33 years

and residing at No. 1, Periyapet Street, Subramaniya

Siva Nagar, Villianur, Puducherry-605 110, do hereby

solemnly affirm and declare as under:

I am the deponent herein. I hereby declare that

so far I have been called as ‘K. Amudha’ vide

Aadhaar Card No. 5440 0687 3055 issued by the

Unique Identification Authority of India.
