‘Iruthayanathan’ in my Office Salary Slip issued by
Animal Husbandry Department, Puducherry vide Bill
No. 22 for the month of March 2015.
Finally, I submit to state and declare that the above
names ‘Irudhayanaathan’, ‘Iruthayanathan’ are one
and the same person, it denotes me and my name
only and further my father name ‘Sandanam @
Joseph’ and ‘Santhanam’ are one and the same person
it denotes my father and his name only.

I have converted from Hindu community to
Muslim community. I have changed my name from
