That I am the deponent herein and I am well

acquainted with the facts herein and I am the resident

of Puducherry having my residence at the abovesaid

That I am originally named as ‘Selvaraj’ and the

same is evidenced in my Birth Certificate duly

registered

Municipality

vide

Registration No. P/1978/09478, dated 29-12-1978, in

my Aadhaar Card bearing No. 6848 7496 0790 issued

by the Unique Identification Authority of India,

Government of India, in my Electoral Identity Card

bearing No. FBN0277533 issued by the Election

Commission of India, as well as in my Family Ration

Card bearing Serial No. 168316 issued by the

Department of Civil Supplies and Consumer Affairs,

Government of Puducherry.

That I was also used to call and addressed by my

parents, family members close relatives, friends and

all other persons in my native place as ‘Selvam’

which is my calling-name and the same name of

‘Selvam’ is also registered in the Birth Certificate of

my son Giritharan who was born to my wife Vanitha

on 13-6-2009 at Government Primary Health Centre,

P.S. Palayam, his birth is duly registered at Villupuram

Taluk Office, Villupuram vide Registration No. 54,

dated 16-6-2009.

That in this connection, I hereby declare that my

names mentioned in my abovesaid records as

‘Selavaraj’ and in my son ‘Giritharan’s Birth Certificate

as ‘Selvam’ are belong and refer to myself only and

not any other person.

The above deposition is true and correct to the

best of my knowledge and belief, and nothing

materials has been concealed thereon.

affirm and state as follows:

I am the deponent herein and I am well acquainted

with the facts of my deposition.

I submit that my present name is ‘N. Marimuthu’.

I submit that I have been advised on numerological

basis to call and change my name as N. Tamilselvi’

instead of ‘N. Marimuthu’.

Henceforth, I would be called and known

as ‘N. Tamilselvi’ only.

I take oath and solemnly declare that the above

information furnished by me is correct and that I have

not concealed or misrepresented any facts on the

above matter.
