Puducherry-605 011, do hereby solemnly declare and

affirm on oath as follows:

I state that I am the deponent herein and I am well

aware of the facts of this deposition.

I state that my son namely ‘V.M. Vishal Ajay’ was

born to us i.e., N. Vijaymurugan and V.M. Latha on

28-5-2007 at Government Maternity Hospital,

Puducherry which was duly registered at Puducherry

Municipality under Registration No. M/2007/07939,

dated 30-5-2007.

I state that in his Birth Certificate, his name has

been mentioned as ‘Vishal’ and as ‘V.M. Vishal’ in his

Aadhaar Card No. 8510 5125 8325 issued by Unique

Identification Authority of India and in his L.I.C Bond

issued by Life Insurance Corporation of India, and

the said name is mentioned in Tamil as ‘’ in

our Family Ration Card No. 339164 issued by the

Department of Civil Supplies and Consumer Affairs,

Puducherry.

I further state that now, the abovesaid name shall

be changed as ‘V.M. Vishal Ajay’ as per numerology

and also he shall use always his name as ‘V.M. Vishal

Ajay’ in all purposes in future.

I state that the names ‘Vishal’, ‘V.M. Vishal’, ‘’

and ‘V.M. Vishal Ajay’ are refer to one and the same

person i.e., my son only.

I state that my son’s correct name and his name

henceforth be known as ‘V.M. Vishal Ajay’ only.

I further state that I made this affirmation in lieu

of proof of Identity of my son’s names.

Hence this sworn-in affirmation may kindly be

accepted to this effect.

I take oath and solemnly declare/affirm that the
