Villianur Commune Panchayat within ten days from the date of publication of this notice in the official gazette.

The objections so received within the specified period will be considered by this Commune Panchayat.

I, Latchoumy, daughter of

Devadasse and wife of

Mounissamy, Hindu, aged

about 54 years, residing at

No. 1, Fourth Cross (East), Tagore Nagar, Lawspet,

Puducherry-605 008, do hereby solemnly affirm and

sincerely state as follows:

1. I am the deponent herein and I state that I was

born on 14-11-1961 to my parents namely Devadasse

and Parimala at Puducherry and my birth was

registered before the competent authority.

2. I state and declare that I am also known and

called as ‘Lakshmi’ by my family members.

3. I state and declare that my name is entered as

‘Latchoumy’ in my Birth Certifiate and my Marriage

Certificate.

4. I further state and declare that my name is

entered as ‘Lakshmi’ in my Voter’s Identity Card,

PAN Card, Aadhaar Card and my Family Ration

Card (in Tamil).

5. I further state and declare that the above names

‘Latchoumy’ and ‘Lakshmi’ refer to one and the same

person i.e., myself.

I swear this affirmation for the proof of my above

two names are one and the same person i.e., myself

only.
