Order and Decree (relating to legal heirs to my

deceased father), dated 6-3-1984 issued by the Court

That my name is mentioned as ‘Selvam’ in

my Voter’s Identity Card No. PY/01/015/042355.

That my name is entered as ‘Selvam’ in my Indian

Passport No. K 2236322 issued by the Regional

Passport Office, Chennai.

That my name is entered as ‘Selvam’ in my

Driving Licence No. 990/VPM/89 issued by the

Assistant Licensing Authority, Transport Department,

Puducherry.

That my name is mentioned as ‘Selvam’ in

my Family Ration Card No. 049185 issued by the

Department of Civil Supplies and Consumer Affairs,

Puducherry.

I declare that the aforesaid names are referring one

and the same person i.e., myself.

By this affidavit, I declare that I shall at all times

hereafter in all records, deeds and writings and in all

proceedings, dealings and transactions private as

well as public and upon all occasions whatsoever use

and sign the name of ‘Selvarassou alias Selvam’

accordingly.
