CHANGE OF NAME

I, Selvi, wife of Venugopal, Hindu, aged 32 years,

residing at No. 32, Sagayamadha Pada Salai, Govindasalai,

Puducherry, do hereby solemnly affirm and declare as under:

I declare that I was born on 25-6-1983 at Pillaithottam,

Puducherry and the said birth was duly registered. In

the Birth Certificate, my name has been entered as

‘Selvi’. The said name has been entered in my

Electoral Identity Card, Aadhaar Card. However, in

my Family Ration Card, my name has been entered as

‘Radha @ Selvi’.

No legal responsibility is accepted for the publication of advertisement regarding change of names and other private

notifications in the Gazette. Persons notifying the same will remain solely responsible for the legal consequences and

also for any other misrepresentations etc.

Hence, I further declare that both the names of

mine ‘Selvi’ and ‘Radha @ Selvi’ refer to one and the

same person i.e., myself only.

I have not concealed any material facts.
