and residing at No. 6, V. T. C. Road, Thirubuvanai

Periyapet, Puducherry-605 107, do hereby solemnly and

sincerely affirm as follows:

I am the deponent herein. I am well aware of the

facts and statement of my affirmation. I swear this

affirmation on behalf of my minor daughter ‘Li Na’

to state that I have made my minor daughter to

embrace her religion from Hinduism to Buddha

Dhamma.

I state that my minor daughter was born us as a

Hindu and her registered name is ‘Li Na’. The said

statement of mine is substantiated through her

Certificate of Birth issued by the Oulgaret Municipality,

registered with it under Registration No. E/2011/1743.

I state that presently I have made my minor

daughter ‘Li Na’ to embrace her religion from

Hinduism to Buddha Dhamma without any coercion

or undue influence from anybody and only with faith

on Buddha Dhamma through a ceremony conducted

by the Member of Triratna Bauddha Sanga. Tamil Nadu

on 14-4-2014 at Tamizh Sangam, Puducherry, through

Vihara, a registered trust. That my minor daughter ‘Li

Na’ would henceforth be known and identified as

Buddhist only.

I state that what are all stated by me in the above

paragraphs are true and correct to the best of

my knowledge,
