I, T.S. Prema, wife of T.M. Sunil Khanna, aged 36 years
and residing at No. 36, 4th Cross, Anitha Nagar,
Mudaliarpet, Puducherry-605 004, do hereby solemnly
declare on oath as follows:
I declare that I am a permanent resident of
Puducherry. My name is entered as ‘Prema’ in
my Family Ration Card, Aadhaar Card, Bank Passbook,
Electoral Identity Card and in all my other records.
However, in my Transfer Certificate and Conduct
Certificate, my name is entered as ‘Maria Prema’.
I declare that my name is ‘T.S. Prema’ only and

‘Sundarrajan’ are one and the same refer to my father
only.
