state as follows :

I am the deponent herein. I state that I was born

on 22-6-1975 at Sulthanpet, Puducherry, my birth has

been duly registered before the Villianur Commune

Panchayat vide Registration No.V/1975/00329. In my

Birth Certificate, my name is entered as ‘Mariambivy’.

I further state that the same name is entered in my

Marriage Certificate, No. 42/92, dated 25-10-1993,

in Tamil as ‘’.

I further state that after my marriage I have changed

my name as ‘Menaga’. The same name is entered

in my Aadhaar Card, Voter’s Identity Card and

in my Family Ration Card. In my Family Ration Card

my name is entered in Tamil as ‘’. I have

obtained Certificate of Identity from the Principal

District Munsif C o u r t at P u d u c h e r r y, d a t e d

2 9 -1 -2 0 1 5 i n C.M.P. No. 23/2015, my name is

mentioned as ‘Menaga @ Mariambivy’. The abovesaid

my names ‘Menaga’, ‘’, ‘Mariambivy’ and

‘Menaga @ Mariambivy’ are referred one and the

same person.

I do affirm that I shall hereafter be known only

as ‘Menaga’ as my name.

This affidavit is to be effect from the publication

of gazette in this regard.

I do hereby declare that what are all stated above

is true to the best of my knowledge, belief and

information and nothing material has been concealed

thereon.
