CHANGE OF NAME

I, Krishnaveni Pothabathula, daughter of Pulleswara

Rao and wife of Seru Veeranna Naikar, residing at Door

No. 03-01-02, Boat Street, Yanam, temporarily came down

to Puducherry do hereby most solemnly affirm on oath

and state as follows:

1. I am to state that I was born on 14-7-1988 at

1-91, Guthulavari peta, Kommanapalli, Mummidivaram,

E.G. District, Andhra Pradesh, to my parents namely

Pothabathula Pulleswara Rao and Pothabathula

Suryakala.

2. I was married Seru Veeranna Naikar, son of

Seru Satyanarayana and Suryakantham of Yanam,

on 25-2-2015 at Antarvedi, Andhra Pradesh,

according to Hindu relegion and then custom, the

marriage was registered before the Sub-Registrar,

Yanam.

3. I am to state that till the date of marriage I have

been called as ‘Krishnaveni Pothabathula’ by my

parents, friends and all my relatives. That after my

marriage, according to Hindu

relegion and

custom I relinquished my parents surname

‘Pothabathula’ and acquired the Surname of my

husband i.e., ‘Seru’. As such henceforth my name will

be ‘Krishnaveni Seru’ instead of ‘Krishnaveni

Pothabathula’.

4. Henceforth, my name will be ‘Krishnaveni Seru’.
