I state that my name has been entered as ‘Adlin’
in my Birth Certificate issued by Kuzhithurai
Municipality, Local Administration Department,
Tamil Nadu under Registration No. 3010279, dated
18-2-1993.
I state that my name has been entered as ‘Adlin Hannah’
in my Family Ration Card No. 189651, Aadhaar Card

I state that I am always use to write and sign my
name as ‘Adlin Hannah’ in all my records, deeds and
writings and in all proceedings, dealings and
transactions, and I shall hereafter also to use this
same name for all transactions whatsoever.
I hereby declare that my above referred two names
viz., ‘Adlin’ and ‘Adlin Hannah’ are one and the same
that is referring myself only.
The above deposition is true and correct to the
best of my knowledge and belief and nothing material
has been concealed thereon.

‘Stellamary’ as my name in place and in substitution
of my former name.

Municipality, in which his name is mentioned without
any initial and (ii) in my above mentioned Aadhaar

That my father is also known and called by the
same name but, with different spellings as
‘Parangusam’ as mentioned in the above mentioned

my name is mentioned as ‘Muruvammal @ Usha’.
But, my name is mentioned in my Family Ration
Card and in my Savings Bank Passbook of Indian
Overseas Bank as ‘Murugammal @ Usha’.
Hence, these names ‘Mourouvammalle’, ‘Muruvammal
@ Usha’ and ‘Murugammal @ Usha’ are refer to one
and the same person i.e., myself only.

spelling

passed by the III Additional District Munsif at
Pondicherry in the suit under O.S.No. 543/2005,
said my father’s name has been mentioned as

Therefore, I do hereby state that the said name
‘Vaitinadane’ as found in the birth record of
my father and the said name ‘Vaithiyalingam’ as
found in my said EPIC and Aadhaar Card, and the
said name ‘Vaitinadin @ Vaitilingam’ as found in the
said Judgment and Decree are refer, identify, relate
to one and the same person that is my deceased
father only.
I state that what are all stated in the above
paragraphs is true and correct to the best of
my knowledge, belief and information.
