affirm on oath as follows:

1. I am the deponent herein.

2. I submit that my name is mentioned as

‘G. Seenuvasan’ in my School Transfer Certificate

issued by the Department of School Education,

Government of Tamil Nadu in Serial No. 19,

PAN Card No. FOUPS0639A, Aadhaar Card

No. 2123 4993 0748, Indian Bank, Savings Account

No. 6218186393 and in my Voter’s Identity Card

No. MVM0260034, I submit that my name is

mentioned as ‘Srinivasan’ in my son Jeevanandham’s

Birth Certificate in Registration No. J/2006/08597 and

in my daughter Gobika’s Birth Certificate in

Registration No. J/2010/07490. I submit that my name

is mentioned as ‘’ in my Family Ration Card

No. 441076. Thus, all the names ‘G. Seenuvasan’,

Srinivasan’ and ‘’ are one and the same.

3. Now, I changed my name as ‘G. Seenuvasan’ for

aforesaid reasons and I shall hereafter be known

by this name.

4. I hereby declare that I shall at all times hereafter

in all records, deeds and writings and in all

proceedings, dealings and transactions, private as

well as upon all occasions whatsoever use and sign

the name of ‘G. Seenuvasan’ as my name in place and

in substitution of my former name.
