previously called Appou Nair (old name), residing at old

No. 45, new No. 51, Thottikara Street, Kanuvapet,

Villianur, Puducherry-605 110, solemnly declare:

1. That for and on behalf of myself and remitter

issue wholly renounce/relinquish and abandon

the use of my former name/surname of ‘Appou Nair’

(old name) and in place thereof, I do hereby assume

from this date the name/surname ‘Appou’ (new name)
