Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973,

objections, if any, to the setting up of the above industry are invited to reach the Commissioner, Nettapakkam

Commune Panchayat within ten days from the date of publication of this notice in the Official Gazette.

The objections, so received within the specified period will be considered by this Commune Panchayat.

CHANGE OF NAME

I, Sumaiya, daughter of

Sankar and Sathiyavani,

aged 21 years, residing at No.45, Akbar Street, Shanthi Nagar,

Lawspet, Puducherry-605 008, do hereby solemnly and

sincerely affirm on oath and declare as follows:

I state that my maiden name is ‘Poongothai’ and

the same is evidenced in my Birth Certificate issued

by the Puducherry Municipality under Registration

No. C/1995/01977, Elector’s Photo Identity Card

No. JPD0061283 and in my Family Ration Card

No. 357998.

I state that my name is mentioned as ‘Poongithai’

in my Driving Licence No. PY01 20140010366.

That I have now changed my name as ‘Sumaiya’

following by the convertion of my religion from

Hindu to Islam on 16-3-2016, (Certificate issued by the

South India Isha-Athul Islam Society, Serial No.73/2016).

I have changed my former Hindu names viz.,

‘Poongothai’ and ‘Poongithai’ into Islamic name

‘Sumaiya’ and hereafter, I shall be known by this new

name in all affairs of my life.

I hereby declare that the above referred three

names viz., ‘Poongothai’, ‘Poongithai’ and ‘Sumaiya’

are denote one and the same person that is referring

myself only.

Further, I hereby declare that I shall at all times

hereafter in all records, deeds and writings and in all

proceedings, dealings and transactions, private as well

as upon all occasions whatsoever use and hereafter,

I shall always use and sign the name ‘Sumaiya’ as

my name in place and in substitution of my former name.

The above deposition is true and correct to the

best of my knowledge and belief and nothing material

has been concealed thereon.
