Puducherry, do hereby solemnly affirm and sincerely

I am the deponent herein and I know all the facts
of my deposition.
I depose that my male child was born on 22-4-2002
and his birth was registered before the Puducherry
Municipality under Registration No.C/2002/00955,
dated 29-4-2002 in that Birth Certificate, the child’s
name is mentioned as ‘Mohamed Shabbeer @
Mohamed Javeed Basha’.
Further, I depose that in my son’s Aadhaar Card
under No. 5431 5798 7958, his name is entered as
‘Mohamed Javeed Basha’. In my Family Ration Card
No. 070872 issued by the Department of Civil Supplies
and Consumer Affairs, Puducherry, my son’s name is
written as ‘ (Mohamed Javeed
Basha)’. The name registered in Primrose School
in Oulgrate, Puducherry is Mohamed Javeed Basha’.
So, the names ‘Mohamed Shabbeer @ Mohamed
Javeed Basha’ are refer to one and the same person
of my son only. Hereafter, he will be called
as ‘Mohamed Javeed Basha’ only.
I have not concealed any material facts.
