AFFIDAVIT

By this deed, I, the undersigned, Mohamed Dawood,

born on 3-4-1982 at Akkaravattam, Karaikal District,

the holder of Aadhaar Card No. 7352 6076 8862, son

of Irudayan and Samboornamary, formerly called as

David, residing at No. 15, Madha Kovil Street,

Nadukalampet - 609 604, Neravy Commune, Karaikal

District, Puducherry State, do solemnly declare :

That for and on behalf of myself and my parent

and remitter issue wholly renounce/relinquish and

abandon the use of my former name/surname of

‘David’ and in place thereof, I do hereby assume

from this date, the name/surname ‘Mohamed

Dawood’ and so that I and my parent and remitter

issue

may

hereafter

be

called

known

and

distinguished not by my former name/surname but,

assumed name/ surname of ‘Mohamed Dawood’.

That for the purpose of evidencing such

my determination declare that I shall at all times

hereafter in all records, deeds and writings and in

all

proceedings, dealings and transactions

private as well as upon all occasions whatsoever use

and the sign the name of ‘Mohamed Dawood’ as my

name/surname in place and in substitution of my

former name/surname.

That I expressly authorised and request all

persons in general, relatives and friends in

particular at all times hereafter to designate and

address me, my parent and remitter issue by such

assumed name/ surname of ‘Mohamed Dawood’

accordingly.

In witness whereof, I have hereunto subscribed

my former name of ‘David’.

this 25th day of November 2016.
