Hindu to Muslim, as such I have abandoned my Hindu

religion name ‘Purushothaman’ and assumed the

Muslim religion name ‘Zayn Ali Syed’. Henceforth,

I shall be known and identified by the assumed name

‘Zayn Ali Syed’, only for all purposes.

I state that what are all stated above is true and

correct.

T hat my name ‘Cannap irane’ as mentioned

in my Birth Certificate found in my daughter and

son’s Birth Certificates, in my Voter’s Identity Card,

Aadhaar Card, PAN Card and Office Identity Cards,

Family Ration Card (in tamil) and in my Bank

Passbook, etc.

That my name also spelled as ‘Kannabiran’ in

my S.S.L.C Book and Mark Sheet, School

Transfer Certificate, Marriage Certificate and in my

Driving Licence, etc.

That I have sworn this affirmation to identify

my names with different spelling and to declare that

both the names ‘Cannapirane’ and ‘Kannabiran’ are

refer to one and the same person that is myself only.

And, I hereby declare that what are all stated

above are true to the best of my knowledge, belief

and information, and noting material concealed therein.

state on oath as follows:

I am the deponent herein. I state that as per my

son’s Birth Certificate registered under Registration

No. C/2002/03111 in Pondicherry Municipality

on 10-12-2002, his name is mentioned as ‘Priyan

alias Sabarish’.

Further, I state that in my son’s Aadhaar Card

bearing No. 4547 6446 9944 issued by the Unique

Identification Authority of India, his name is entered

as ‘Priyan alias Sabarish’. In my Family Ration Card,

my son’s name is wrongly mentioned as ‘Marian’

(in Tamil).

I state that now, I have changed my son’s name

as ‘Priyan Sabarish’ for numerological and sentimental

reasons. My son shall be known and identified by

the name ‘Priyan Sabarish’ only for all purposes.

I state that the facts stated above are true and

correct to the best of my knowledge, belief and

information.
