Periyababusamudram Post, Puducherry-605 102,

do hereby solemnly affirm and sincerely state as

follows:

I state that my name is ‘Rajeshkannan’ and the

same is evidenced in my Birth Certificate issued by

the Mannadipet Commune Panchayat, Puducherry

under Registration No. T/1984/00073, dated 2-3-1984,

Aadhaar Card No. 4305 6691 1713, Voter’s Identity

Card, Driving Licence and in my Bank Passbook.

I state that I am also known by the name ‘’

and the same is evidenced in my Family Ration Card

No. 326069.

Further, I state that both the names ‘Rajeshkannan’

mentioned as ‘Mogane Lalloume’.

In the Transfer Certificate under No. 59362 and in the

Service Certificate of mine under No. 4512/GCP/Estt.-

II/E3/2017/20, my name has been mentioned as

‘L. Mohan’. In the Family Ration Card under

No. 120515, my name has been mentioned as ‘’.

Therefore, I do hereby state that the said names

‘Mogane Lalloume’, ‘L. Mohan’ and ‘’ as

found in the abovesaid documents are refer, identify

and relate to one and the same person that is myself

the deponent herein. I shall henceforth be known

and identified by the name ‘C. Mogane Lalloume’

only for all purposes.
