Meenatchipet, Thattanchavady, Puducherry-605 009,

2. I state that my son ‘Deepak Kumaar’ was

born on 7-1-2013 at Ashwini Maternity Hospital,

Puducherry and his birth was registered before the

Registrar of Births and Deaths, Oulgaret Municipality,

Puducherry under Registration No. O/2013/00014.

3. I state that hereafter, my son will be called

as ‘Deepak Rishi’.

4. I state that the facts stated above are true to

the best of my knowledge, belief and information.

Vysya Bank, Karaikal, my name has been mentioned

as ‘Kumanan N.P’.

In my PAN Card under No. AMSPP8371R,

Partnership Deed, Provisional Certificate and

Memorandum of Deposit of Title Deeds for

M/s. Power Polytech Industries, Karaikal, my name

has been mentioned as ‘N.P. Kumanan’.

Therefore, I do hereby state that the said names

‘Kumanan’, ‘Kumanan Paramasivam’, ‘Kumanan N.P’

and ‘N.P. Kumanan’ are refer, identify and relate to one

and the same person that is myself the deponent herein.

I state that henceforth I will be known and

identified by the name ‘N.P.Kumanan’ only for all

purposes.
