Samipillaithottam, Puducherry-605 008, do hereby
solemnly and sincerely affirm and state on oath as
follows:
I submit that my name ‘Aroquiaradjou’ found in
my Birth Certificate, Registered in the Puducherry
Municipality, under registration No. 58/69 on
4-1-1969, the name ‘Aroquiaraj. M’ found in my
Transfer Certificate issued by the Principal, V.O.C.
Government High School, Cathedral Street,
Puducherry, the name ‘Aroquiaraj. M’ found in my
Secondary School Leaving Certificate issued by the
Secretary, Board of Secondary Education, Tamil
Nadu, bearing Registration No. 154553, the name
‘Aroquiaraj’ found in my Aadhaar Card issued by the
Unique Identification Authority of India, bearing
No. 9823 4151 0122, the name ‘Aroquiaraj’ found
in my Voter’s Identity Card issued by the Election
Commission of India, bearing No. PY/01/021/
111405, and the name ‘M. Aroquiaraj’ found in my
Service Certificate issued by the Executive
Engineer-VII, Electricity Department, Government
of Puducherry are refer to one and the same person
of myself. I do affirm that hereafter, I shall be
known only as ‘M. Aroquiaraj’.
I declare that the abovesaid facts are true to the
best of my knowledge, belief and information.
