I state that my name is ‘Arthi. L’ as mentioned

in the Study Certificate issued by the Principal,

Seventh-Day Adventist Higher Secondary School,

Puducherry.

Further, I state that after my marriage with

‘Rengeethe Jain’, I was called as ‘Arthi Jain’ as

stated in my old Aadhaar Card.

Further, I state that due to numerological

reason, I have changed my name as ‘Aarti Jain’ as

mentioned in my Mark Statement issued by the

Central Board of Secondary Education, Elector’s

Photo Identity Card and in my new Aadhaar Card.

Further, I state that the names viz., ‘Arthi. L’,

‘Arthi Jain’ and ‘Aarti Jain’ are denote, refer to one

and the same person i.e., myself.

Further, I state that now, I decided to abandon the

names ‘Arthi. L’ and ‘Arthi jain’ and hereinafter,

I shall be known and called as ‘Aarti Jain’ only.

‘Sivapragasam’ and ‘Siva’ are refer to one and the same

person i.e., myself only.
