this notice in the Official Gazette.

I, Poonkodi, daughter of Saravanan and Premlatha,

date of birth 14-3-1996, aged 21 years, Converted Muslim,

residing at No. 3, Jeevarathinam Street, Rajiv Gandhi

Nagar, Veerampattinam, Puducherry-605 004, do hereby

solemnly affirm and sincerely state as follows:

I state that my name has been mentioned as

‘Poonkodi’ in my Birth Certificate issued by the

Pondicherry Municipality, Aadhaar Card and in my

Elector’s Photo Identity Card.

Further, I state that my name has been mentioned

(in Tamil version) in my Aadhaar Card No. 7455 6469

6381 as ‘’ and the same name has been

mentioned as ‘’ in my Elector’s Photo

Identity Card No. ZQJ0074468. In my grandfather’s

Family Ration Card No. 377848, my name has been

mentioned as ‘’.

I declare that all the names mentioned in the

aforesaid Certificates are denote and refer to one

and the same person that is myself only.

Further, I state that I was a Hindu by birth,

recently, I have embraced Islamic faith. Upon

Conversion of my religion, I had renounced my birth

name ‘ (Poonkodi)’ and assumed the

Muslim name of ‘S.  (Ameera).

Further, I declare that hereafter, I will be known

and called by the name ‘S.  (Ameera) only.
