convertion to Muslim, I have changed my name

as ‘Farvin’ (in Tamil). After conversion to Islam, I

have relinquished my birth name ‘Santhi’ and

assumed the new name ‘Farvin’.

Henceforth, I will be known, called and identified

by my assumed name as ‘Farvin’.
