Kuyavarpalayam, Nellithope, Puducherry-605 005,
do hereby solemnly and sincerely affirm and state as
follows:
I am the deponent herein and I am well aware
of the facts of my deposition.
I state that in my Birth Certificate and School
Transfer Certificate, my name is mentioned
as ‘Kalaiselvi’.
I state that in my Aadhaar Card, in my
daughter’s Birth Certificate and in my son’s Birth
Certificate, my name is mentioned as ‘Vijaya’.
I solemnly affirm and declare that the names
‘Kalaiselvi’ and ‘Vijaya’ are refer to one and the
same person that is myself and hereinafter, I shall be
called by the name ‘Kalaiselvi @ Vijaya’.
I state that what are all stated above are true to
the best my knowledge, belief and information and
nothing material facts have been suppressed herein.

I state that my name has been entered as ‘Durai’
in my Voter’s Identity Card No. GKY0442251.
I state that my name has been entered as
‘Suppuraya alias Durai’ in my Aadhaar Card No.
8930 9542 4606 and ‘’ in my
Family Ration Card No. 448605.
I hereby declare that the above referred all the
names viz., ‘Suppuraya Pillai’, ‘Durai’, ‘Suppuraya
alias Durai’ and ‘’ are refer
to one and the same person that is myself only.
Now, I have adopted my Family name ‘Yadav’
along with my name ‘Suppuraya Pillai @ Durai’ and
assume my name as ‘Suppuraya Pillai @ Durai Yadav’.
Hereafter, I shall be known by this new name.
I state that hereafter, I will always use to write
and sign my name as ‘Suppuraya Pillai @ Durai
Yadav’ in all records, deeds and writings and in all
proceedings, dealings and transactions whatsoever.
