AFFIDAVIT
I, Peetha Gowri Devi, wife of P. Srinivas @ Srinivasarao
(late), aged 35 years, residing at Door No. 03-06-002,
Thyagaraja Street, Yanam, do hereby solemnly affirm
and state on oath as follows:
I state that I am an affiant and I am well aware of
this affidavit.
I state that my daughter ‘Peetha Sarika’ born on
20-5-2002 at Yanam. Her name is mentioned and
registered as ‘Peetha Sarika’ in the Birth Records of
Registrar of Births and Deaths, Yanam Municipality,
Yanam and in my Aadhaar Card bearing No. 6302 3068
0883. Whereas, in my Family Ration Card bearing
No. 000087, her name is mentioned as ‘Sarika’
(in Telugu). But, her name is mentioned as ‘Peetha
Durga Lakshmi Sarika’ in her Study Certificate
F. No. 4-2/JNVMY/2017-18, dated 20-9-2017.
I submit that the names ‘Peetha Sarika’, ‘Peetha
Durga Lakshmi Sarika’ and ‘Sarika’ are refer to one
and the same person i.e., my daughter only.
I intend to continue my daughter’s name as ‘Peetha
Sarika’. Therefore, for the purpose of evidencing such
determination, I declare that she shall at all times
hereafter, in all the records, deeds and writings and
in all proceedings, dealings and transactions, private
as well as public and upon all occasions whatsoever
use and sign the name of ‘Peetha Sarika’ as her name.
That I expressly authorize and request all persons
at all times hereafter, to designate and address my
daughter by such assumed name of ‘Peetha Sarika’
accordingly.
Hence, I am swearing this affidavit by me in this
regard, since, my daughter is a minor who is aged
15 years.
