I, Ancy Aravindan, daughter of P.V. Aravindan,

aged 35 years, residing at Sivakavyam, Palloor, Mahe,

do hereby solemnly affirm and state as follows:

by the Mahe Municipality, Mahe, my mother’s name

identify and denote one and the same person that is
