I, M. Valarmathy, mother of Abhinaya, aged 33 years

and residing at No. 72, Bharathidasan Street, Muthialpet,

Puducherry-605 003, do hereby solemnly and sincerely

affirm on oath and declare as follows:

I state that my minor daughter’s name has been

mentioned as ‘Abhinaya’ and the same is evidenced

in her Birth Certificate issued by the Oulgaret

Municipality under Registration No. K/2014/00546,

dated 1-8-2014.

I state that for and on behalf of my minor
