Elector’s Photo Identity Card bearing No. SCO0010934,
dated 9-7-2014, Passport bearing No. M1529460, dated
21-8-2014, Aadhaar Card bearing No. 3100 4631 1450,
dated 30-6-2011, Family Ration Card bearing
No. 106295 (in Tamil) and PAN Card, dated 10-11-1990
as such I am maintaining the said birth name.
Further, I state and declare that due to sentimental as
well as astrological reasons, I have desired to and keep
my name as ‘Marthina’ only.
Therefore, I declare that hereafter, I shall be called and
known as ‘Marthina’ only. Thus, I am swearing/executing
this affidavit in this regard.
