AFFIDAVIT
I, Sellapandi, son of Periyathambi, aged 33 years,
residing at No. 33, South Street,
Thakkalour,
Thirunallar, Karaikal, do hereby solemnly affirm and
sincerely state as follows:
I was a Hindu by birth, my birth name was
‘Sellapandi’, recently, I had embraced Islamic faith.
Upon conversion of religion, I had renounced
my birth name ‘Sellapandi’ and assumed to myself
the Muslim name as ‘Farooj’.
I hereby declare that henceforth, I shall be known,
called, referred and identified only as ‘Farooj’.
Solemnly affirmed and signed before the Notary Public
at Karaikal, on this 2nd day of February 2018.

Extraordinary Official Gazette No. 40 of the 29th
May 2012 of the Government of Puducherry, the
words “Meritorious Sports Persons Quota” shall be
substituted.
