CHANGE OF NAME

I, Karthikesan, son of Thirumeni Azhagar and

Panchavarnam, aged 24 years, converted Muslim,

residing at No. 17, Oppila Maniyar Kovil Street, Keerai

Thottam, Karaikal, do hereby solemnly affirm and

sincerely state as follows:

I was a Hindu by birth. My birth name was

‘Karthikesan’. I had renounced my religion of birth

and embraced Islam on 1-2-2018. Pursuant to

conversion, I have changed my name as ‘Abdur

Rahman ()’.

I hereby declare that I relinquish and give up my

birth name ‘Karthikesan’ and henceforth, I shall be

known, called, referred and identified only as ‘Abdur

Rahman ()’.

Solemnly affirmed and signed before the Notary Public

at Karaikal, on this 24th day of March 2018.
