I, J. Kamalakannan, son of Janakiraman, Hindu, aged
about 45 years and residing at No. 4, Sivaranthagam
Road, Ariyur, Puducherry-605 110, do hereby solemnly
and sincerely affirm on oath as follows:
I am the deponent herein. I submit that my name
is mentioned as ‘Kamalakannan. J’ in my Transfer-
cum- Conduct Certificate in Serial No. 2012-7261,
under Registration No. 29801028 issued by the
Muthiah Polytechnic College, Annamalai Nagar.
I submit that my name is mentioned as
‘J. Kamalakannan’ in my Aadhaar Card under

I submit that my name is mentioned as ‘Kamalakannan’

I submit that my name is mentioned as ‘’
in my Family Ration Card under No. 109191.
Thus, all the names ‘Kamalakannan. J’, ‘J. Kamalakannan’,
‘Kamalakannan’ and ‘’ are refer to one and
the same person that is myself only.
Hereafter, I shall be known only by the name
‘J. Kamalakannan ’.
I hereby declare that I shall at all times, hereafter
in all records, deeds and writings and in all
proceedings, dealings and transactions private as
well as upon all occasions whatsoever use and sign

as my name in place and in substitution of my former

CHANGE OF NAME

I declare that I am a permanent resident of
the abovesaid address. My daughter by name
‘A. Harinie’, daughter of P. Angalane, who was born
on 10-1-2004 at Maternity Hospital, Puducherry
was registered at the time of birth instead of her
name ‘S. A. Harinie’ due to ignorance and now
rectified my daughter’s name as ‘S.A. Harinie’ as
per legal procedures by publishing in the Official
Gazette of Puducherry.
This affidavit is made in lieu of the aforesaid
facts and I have not concealed any material facts.
