AFFIDAVIT
I, M. Vaishnavi Prabha, aged 30 years, daughter of
M. Mahendran (father) and V. Mohana (mother). I was
born on 29-3-1988 at Maternity Hospital, Puducherry.
My father M.Mahendran and my mother V. Mohana
are residing at No. 31, Pillaiyar Koil Street, Pillaiyarkuppam
Village, Bahour Commune, Puducherry-607 402.
At my age 23 i.e., 1-12-2011, M. Raguraman,
who is a n e ld e r b ro t h er o f my fa t h er an d
R. Jagadhambal, who is the elder sister of my
mother are residing at No. 87, Thendral Street,
Sudhana
Nagar
Extention-1,
Murungapakkam,
Puducherry-605 004 legally adopted me and the same
was agreed by me. They are blood relatives to me
and no child was born to them till the date of
adoption to M. Raguraman and R. Jagadhambal. On
1-12-2011, the adoption was registered as Adoption
Deed before the Office of the District Registrar,
Puducherry in Document No. 1385 of 2011, Book
No. IV Page of 1 of 6.
Further, I have filed a Suit before the II Additional
District Munsif Court in Puducherry in O.S. No. 1476
of 2015 and suit was taken for trial on 12-12-2015 in
LAC No. 24852 of 2015 and pronounced a
Judgment, dated 12-12-2015 with the following
observation.
“Therefore, as per the settlement arrived between
the parties before this Lok Adalat, it is declared that
the registered Adoption Deed, dated 1-12-2011
registered under Document No. 1385/2011 before
the District Registrar, Puducherry is valid”.
Based on the above registered Deed and Judgment
of Lok Adalat, I have been changed my name such
as ‘R. Vaishnavi Prabha’, daughter of M. Raguraman
and R. Jagadhambal instead of ‘M. Vaishnavi Prabha’
daughter of M. Mahendran and V. Mohana.
The application is filed without any compulsion
and the circumstances narrated are true and on my own
willingness.
