AFFIDAVIT
I, Tenemojy, daughter of Sinouvassin and wife of
Muthaiyan alias Sithanandham, aged 62 years and
residing at the House bearing Door No. 46, Main Road,
Chellan Nagar, Puducherry-605 011, do hereby
solemnly and sincerely affirm to whomsoever it may
concern, as follows:
I am the deponent herein. I state that in my
Certificate of Birth issued by the Registrar of Births
and Deaths, Pondicherry Municipality, registered
with it under Registration No. P/1956/00162 and in
my Passport issued by the Regional Passport Office,
Chennai, under No. J8534708, my name has been
mentioned as ‘Tenemojy’.
Further, I state that in my Elector’s Photo Identity
Card under No. PY/01/020/066049 and in my
Aadhaar Card under No. 4052 5261 0666, my name
has been mentioned as ‘Thenmozhi ()’.
In the Family Ration Card under No. 107080, my
name has been mentioned as ‘’.
Therefore, I do hereby state that, all the abovesaid
names are refer, identify and relate to one and the
same person, that is myself, the deponent herein.
I state that, hereafter, I shall be known and identified
by the name with spelling as ‘Tenemojy ()’
for all purposes.
I state that what are all stated in the above
paragraphs is true and correct to the best of my
knowledge.

CORRIGENDUM
In partial modification of the G.O. Ms. No. 19/LAS/
A1/2018, dated 1-10-2018 of the Local Administration
Secretariat, Puducherry, published under Part-II of
Extraordinary Gazette No. 54, dated 5th October, 2018,
the following corrections are made:
(1) In page 814, para 3, line 9, instead of “with
effect from 2nd October, 2018”, the words
“notionally with effect from 1-1-2016 and the
monetary benefit with effect from 2nd October,
2018” shall be substituted.
(2) In page 815, para 6, line 1 and 2, “from
2nd October, 2018”, the words “notionally with effect
from 1-1-2016 and the monetary benefit with effect
from 2nd October, 2018” shall be substituted.

Extraordinary Gazette No. 53, dated 5th October, 2018,
the following corrections are made:
(1) In page 781, para 1 (2) instead of “They shall
come into force on and from the 2nd day of
October, 2018”, the line “They shall be deemed
to have come into force notionally with effect
fr o m 1 -1-2 016 and with monetary b enefit
from 2nd October, 2018” shall be substituted.
(2) In pages 782 to 791, wherever the words
“2nd day of October, 2018” appears, the same shall
be substituted with the words “1st day of January, 2016”.
(3) In page 791, para 11, line 1 instead of year
“2018”, the year “2016” shall be substituted.
