I am the deponent herein and I am well known the

facts of my deposition. My name is mentioned as

‘Raghunath Baliyar Singh ()’ in my

Aadhaar Card, Family Ration Card and in my Indian

Passport.

But, in the sale Deed Document, Document

No. 2650/1991, dated 3-10-2010 issued by the

Sub-Registrar Office, Oulgaret, Puducherry, my

name as ‘’ and also my name mentioned as

‘Ragunath’ in my Water Bill issued by the Public

Works Department, Puducherry.

Hence, I hereby declare that all the names

‘Raghunath Baliyar Singh’, ‘Raghunath’ and ‘Ragunath’

are refer to one and the same person i.e., myself

only.

That, my correct and proper name is ‘Raghunath

Baliyar Singh’ only.

I am making this affidavit for the proof of my

name’s identity and to be produced before the

concerned authorities.
